Welcome to FLCHS Athletics! In an attempt to help all student-athletes with FHSAA eligibility, the following guide was created. All student-athletes must have an FHSAA profile. The directions below will link student-athletes directly to all mandated forms and courses.
The mandatory forms to be uploaded include:
The entire EL2 Preparticipation Physical Evaluation (all 4 pages)
Two Diocesan Consent forms (one for medical and one general consent)
Three NFHS video certificates for Sudden Cardiac Arrest, Heat Related Illness, and Concussion EL3 FHSAA Consent forms (all 5 pages)

There should be a link on the FHSAA registration portal to access the NFHS videos that all student-athletes are required to view. If you are having difficulty accessing that link click here: https://nfhslearn.com/courses
Student-athletes will need to make an account. Here are the links for the three required courses (choose Florida next to the $ for each of these FREE courses):

Once student-athletes complete the courses, please upload a copy of each certificate on the student-athlete’s FHSAA profile.
